Item dispatched, Buyer didn't received within timelines, he got mail that makes him afraid stating that item is treated as received if not claimed.
Until the mail comes, we feel that Buyer won't be worrying much. Upon Sellers requests and understanding the situations, buyer will wait for delivery to happen.
But once he got the mail, he gets frightened for his money and item. He calls or writes to eBay, he receives advice to raise refund immediately. He then raises refund.
Seller comes in and explains the buyer and requests to wait for some days. Buyer understands and agrees for keeping money refund on hold. Here, Buyer don't want to take chance by letting money to Seller until he receives item. He also, don't want the get refund immediately as he needs the item.
The real amusement comes here. eBay executives will bend the Sellers to accept the refund and bear loss.
