Its always a good idea to send the order statement with the product as it have all your contact details which will be helpfull for the customer. I usually print all the shipping documents except the shipping declaration. Here is what I do:
Lable - Print on the sticker and paste outside the envelope.
Declaration - Don't Print
Order Statement- Print and put inside the envelope with the item
Manifest - Print all the manifests to get it signed by the pickup guy
